Hematology Oncology Life Center, LLC files each claim for you, unless you request to file your own insurance claims and accepts the assignment of Medicare. It is extremely important that you report any changes in insurance coverage to the front office so that claims will be filed properly.

You will receive a statement at the end of each month. The statement will tell you what charges you have had, tell you what has been filed on the insurance or co-insurance, and any payments that have been made.

Patients with only one health insurance plan will be responsible for payment of 20% deductible and also for any charges not allowed.

Hematology Oncology Life Center will file cancer policies for you. Disability forms and other miscellaneous insurance forms are also completed for you. There will be a nominal charge for completing these forms.
